The Indonesian visa application package must include the following documents:
- The original SIGNED passport with a validity of more than 6 months, (passport must be in good condition, not torn or damaged and has at least ONE visa blank page to affix the visa sticker). A limited-validity passport will not be accepted without a prior Indonesian visa.
- 1 photocopy of ID page of the passport.
- Visa application forms, properly completed online, please click on this link← Please be advised that uploaded photo at the online visa application form must be the same that you would provide us later by mail with other paperwork.
- Two color photographs, recent, of the same drawing, passport format.
- Copy of inbound and outbound travel itinerary or a letter from travel agents, airline, steamship company, confirming the purchase of tickets.
- If you travel with kids (under the age of 18), you will need to attached the following documents:
a. Copy of the child’s birth certificate (notarized, we can do this for additional cost 5 US.
b. A Letter of Consent signed by both parents and notarized by a Notary Public.
c. Copies of a Government issued ID such as passport or Driver’s License of both the parents should be attached.
- Letter of employment, indicating the employment status of the applicant. For retiree, please submit proof of the retirement fund. For students please provide letter of intent from the school stating that the student still enrolled at the school.
- Copy of bank statement, indicating the current applicant's bank account in the last two weeks, or a letter from the bank stating that the applicant has a sufficient fund.
- For Non-US citizen: Please provide a copy of US Permanent Residence/Visa/Valid I-20 from schools/universities.
- Vaccination (Yellow Fewer) is recommended but not mandatory, and is required only if applicant had previously traveled to areas/ countries infected by yellow fever.
- Record of Travel in the last 14 Days.
- Health certificate issued by health authority that stated applicant is fit for travel and free from respiratory infection symptoms
- Visa processing fee. We can arrange this payment on your behalf. In this case we will charge you additionally 5 US (US Bank money order preparation fee) + 4% of the official consular fee as PayPal charge. The payment can be done here: https://washingtondc.wegetvisas.com/node/64
- Prepaid return envelope or we can do it for you for additional cost.
